Adoption and Sterilization Agreement |

| Owner agrees to be personally
responsible for the care, humane treatment and control of this animal.
The yearly cost of caring for an animal is estimated to be between $200 and
$500 per year. Owner certifies that he/she is financially capable of
properly caring for this animal. The HSAC may request an at-home visit
to ensure the well-being of this animal, and Owner agrees to allow HSAC to
visit Owner's home to determine the quality of care of this animal. If
Owner decides to no longer care for this animal, Owner agrees to relinquish
custody and return this animal to HSAC free of any charges of any kind
incurred by Owner including but not limited to: fees, licenses, care
and feeding, medical services and/or procedures. Owner agrees that
this animal cannot be used for any scientific, medical and/or research
experiments. Owner understands that HSAC does not guarantee the health
or temperament of this animal, and agrees to release, to hold harmless, and
to forever discharge HSAC, its employees, directors, agents and
representatives from all claims, demands and causes of action of any kind
whatever arising out of, or in any way connected with any harm or injury to
any person or thing which may be caused by this animal. HSAC
recommends that Owner take this animal to a licensed veterinarian, within
three (3) days from the date of this Agreement, for a complete physical
examination, at Owner's cost, in order to insure its health and health
maintenance. Refunds will be given in the form of a check on the 15th
and 30th of each month. Owner understands that he/she is required to
return this animal to HSAC for sterilization performed by a licensed
veterinarian, and agrees to said procedure, and agrees not to hold HSAC, its
agents and/or representatives responsible in any way for any complications
that may occur during this surgery. Owner understands that HSAC
reserves the right to verify information provided by Owner in this
Agreement, and that falsification of any information contained herein can
cause HSAC to deny adoption of this animal to Owner. Owner agrees that
failure to comply with the terms and conditions of this Agreement will void
same, and Owner agrees to return this animal to HSAC, and forfeit all fees
paid to HSAC in connection with the adoption of this animal.
I HEREBY CERTIFY THAT THE INFORMATION CONTAINED IN THIS AGREEMENT IS TRUE AND CORRECT TO THE BEST OF MY KNOWLEDGE AND BELIEF, AND I HEREBY ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS AGREEMENT AND ACCEPT AND AGREE TO THE TERMS AND CONDITIONS STATED HEREIN.
